People and families referred to our Support Centres have multiple and very complex needs. It is not possible for TMTP or any other one single organisation to meet all the needs someone may have and it is therefore crucial that different organisations work together and work together well.
It is for these reasons that TMTP have put into place an effective 'client navigation team’ who operate an intentional and robust inter-agency referral mechanism. The client navigation team sit in-between our front line service support and a growing city-wide network of strategic partners who provide add-on specialist external support, as required:
Our aim is to improve the overall client experience and work hard to ensure clients move from crisis to independence in the best possible way.
TMTP and our strategic partners meet once a quarter at senior management level to discuss the impact and quality of support that is being provided collectively, and collaboratively address the challenges that arise from time to time.
